Define review triggers
Identify which outputs require approval and which conditions should pause work. Missing information, unexpected responses, and conflicting records should have a defined destination. Do not rely on a general instruction to use good judgment.
Give reviewers enough context
Present the original input, proposed action, relevant source information, and the reason for escalation together. Make it possible to approve, correct, reject, or request more information. Record the decision without requiring reviewers to reconstruct the entire workflow.
Learn from exceptions
Review recurring exception types with the operational team. Some problems call for better inputs or a process change rather than a different model. Update the workflow deliberately, retest changed behavior, and preserve an audit trail of approvals.
